Job summary

AxessPointe Community Health Centers in Akron is seeking a Community Health Worker Student to support at-risk individuals by partnering with community agencies to navigate social services and healthcare systems, promoting healthy behaviors and managing conditions.

Under Nurse Care Manager supervision, the student will perform home visits, enroll clients in Medicaid and a medical home within 14 days, document in eCW, advocate for clients, and assist with planning and follow-up.

Responsibilities

• Work directly in community settings to assist residents and complete CHW training.
• Canvass clinics and community centers to identify at-risk individuals and enroll them in programs.
• Enter home-visit information into electronic health records (eCW) accurately.
• Advocate for clients and liaise with service providers to coordinate care.
• Assist clients with understanding and following medical recommendations.
• Determine client eligibility for services and refer to appropriate resources.
