Job summary

Community Health Service, Inc. (CHSI) in Grafton, ND is seeking a Full-time Community Health Worker to support patients in the community by increasing health knowledge and self-sufficiency through outreach, education, counseling, social support and advocacy.

The role includes coordinating with local partners, helping patients with insurance, transportation, and navigating government services while ensuring HIPAA privacy and teamwork in a nonprofit setting.

Qualifications

• Certified Traditional Health Worker or Community Health Worker.
• Knowledge of HIPAA privacy rules and regulations.
• Excellent oral and written communication skills.
• Valid driver''s license and ability to travel as required.
• Experience in a non-profit health care setting preferred.

Responsibilities

• Assist patients with the insurance application and track through completion.
• Help patients find transportation options to receive services.
• Learn about community partners for resources (food, basic needs, transportation, insurance).
• Assist patients with resolution of issues navigating governmental departments, etc.
• Assist with bringing new resources to the community.
• Performs other duties as assigned.
