Job summary

Community Health Service, Inc (CHSI) in Willmar, MN is seeking a Full-time Community Health Worker to support patients in the community through outreach, education, counseling, social support and advocacy. The role involves coordinating with local partners, ensuring HIPPA compliance, and helping patients access MNsure and transportation to services.

Qualifications

  • Certified Traditional Health Worker or Community Health Worker
  • Knowledge of HIPPA privacy rules and regulations
  • Ability to establish and maintain effective working relationships with staff, patients, and the community
  • Excellent oral and written communication skills
  • Must have knowledge of Microsoft Office (Word, Excel, Outlook)
  • Ability to manage multiple tasks
  • Enthusiasm for working as a member of a team in a rapidly changing environment
  • Ability to work effectively and harmoniously with co-workers
  • Excellent organizational skills
  • Experience in a non-profit health care setting preferred
  • Valid driver’s license and must have the ability to travel as required

Responsibilities

  • Assist patients with the MNsure application. Track and follow‑up through completion.
  • Assist patients to find transportation options to receive services.
  • Learn and become familiar with what community partners are available for our patients (food, basic needs, transportation, and insurance).
  • Assist patients with resolution of issues—navigating governmental departments, etc.
  • Assist medical staff in researching options for the patients (other than medical referrals and medications).
  • Assist with bringing new resources to the community.
  • Perform other duties as assigned.

WHAT DO WE DO?

CHSI is a private, non‑profit organization established in 1973 to serve the health needs of migrant & seasonal farm workers and their family members. Today, CHSI has expanded services and provides mental & physical healthcare to all individuals in our communities. Our services are provided on a more affordable sliding fee scale to those who are under‑insured or un‑insured. We also accept ND and MN Medicaid, Medicare, and most private insurance plans. We currently operate four nurse‑managed clinics, two mobile medical units, and two victim advocacy program centers in Minnesota & North Dakota.

WHO ARE WE LOOKING FOR?

We are on the search for a Full‑time Community Health Worker to join our Willmar, MN team. In a Community Health Worker role at CHSI, you will be responsible for providing assistance to patients in the community by increasing health knowledge and self‑sufficiency through a range of activities such as outreach, community education, informal counseling, social support and advocacy. This will include working with community stakeholders and local and state organizations. Patient confidentiality and knowledge of HIPPA rules is a priority. This position requires a self‑starter that needs very little input to accomplish their work in providing services for our patients.

QUALIFICATIONS:

EDUCATION AND/OR EXPERIENCE:
  • Intermediate level computer skills and ability to use Microsoft Windows, Explorer, Word and Excel
  • Experience in Non‑profit preferred: 1 year

PHYSICAL ABILITIES
  • In an eight hour workday, the employee must be able to bend and crouch occasionally, sit continuously and stand or walk frequently.
  • Employee must be able to lift up to 50 pounds occasionally.
  • Employee must be able to use hands/arms, feet/legs for repetitive fine to large motor skill movements.
