CHSI is a private, non-profit organization established in 1973 to serve the health needs of migrant & seasonal farm workers and their family members. Today, CHSI has expanded services and provides mental & physical healthcare to all individuals in our communities. Our services are provided on a more affordable sliding fee scale to those who are under-insured or un-insured. We also accept ND and MN Medicaid, Medicare, and most private insurance plans. We currently operate four nurse-managed clinics, two mobile medical units, and two victim advocacy program centers in Minnesota & North Dakota.
We are on the search for a Full-time Community Health Worker to join our Grafton, ND team. In a Community Health Worker role at CHSI, you will be responsible for providing assistance to patients in the community by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support and advocacy. This will include working with community stakeholders and local and state organizations. Patient confidentiality and knowledge of HIPAA rules is a priority. This position requires a self-starter that needs very little input to accomplish their work in providing services for our patients.
