Job summary

HealthCare Access Maryland seeks a dedicated RCCC Community Health Worker to provide outreach, care coordination, and resource navigation for individuals returning to Baltimore County, Baltimore City and Anne Arundel County. You will educate clients about health insurance options, assist with benefits enrollment, and link them to essential services.

The role requires on-site work, field visits, and collaboration with a broad network of providers.

Qualifications

• High School Diploma or GED with social services experience; Bachelor's preferred.
• Maryland-specific certifications may be required within stated timelines.

Responsibilities

• Outreach and social support to clients and families, pre- and post-release.
• Coordinate care and connect clients to community resources in Baltimore area.
