Job summary

Health Care for the Homeless seeks a Community Health Worker in Maryland to reengage clients and help ensure ongoing access to services, housing, and specialty care. Working with an interdisciplinary team, you will identify barriers and provide health education and risk reduction interventions.

You will assist with in-home and community supports, coordinate transportation to appointments, and document interventions promptly to support coordinated care and housing access.

Qualifications

• High school diploma or GED required; Associate or Bachelor’s preferred.
• Valid driver’s license with reliable transportation preferred.
• Able to attain Maryland Community Health Worker Certification within six months of hire.
• Experience serving vulnerable populations in community settings preferred.

Responsibilities

• Reengage clients connected with Health Care for the Homeless to improve engagement with services.
• Provide supportive services in-home and in the community, including reminder calls and coaching on daily living activities.
• Escort clients or coordinate transportation to off-site appointments to support adherence to care plans.
• Problem-solve with clients to overcome barriers and achieve identified goals.
• Serve as the eyes and ears of the care team and share information for care planning.
• Navigate housing resources and the Baltimore City Coordinated Access system and related services.
• Participate in an integrated, multidisciplinary team to implement treatment plans and attend case conferences.
• Accurately document all interventions and encounters per agency policy.
