Job summary

Family Health Center seeks a Community Health Worker/Patient Navigator to outreach, recruit, and support patients to engage in care and care management services within the population health team.

The CHW will educate and advocate for patients, assist with accessing services, and follow up on external referrals while coordinating care with primary teams.

Qualifications

• High school diploma or GED required; Bachelor’s degree preferred.
• Two years relevant work or college-level internship experience required; proven ability to perform patient interviews and coordinate with internal/external services.
• Bilingual in English and another language appropriate to patient population; CHW Core Training within first year of employment.
• Community Health Worker or Medical Interpreter certificate preferred.

Responsibilities

• Supports the care team with patient follow-up, reminders, and addressing barriers to care.
• Documents all services as required and assists in meeting program requirements.
• Collects and analyzes data related to the project; works with data management staff on data collection.
• Provides outreach and meets with patients to explain services and coordinates the treatment plan.
• Facilitates referral process for patients using Healthify, E-referral, Community Help, and other tools.
• Advocates for patients regarding transportation, interpretation, health insurance, and FHCW services.
• Maintains strong organizational and communication skills to act as hub of information across departments.
• Performs other duties as assigned.

This Community Health Worker will provide support to the Primary Care Teams by assisting with programs, initiatives, and day-to-day operations that promote quality patient care and improve health outcomes across the community.

The Community Health Worker/Patient Navigator (CHW) will work to outreach, identify, recruit, and support patients to engage in care and/or care management services; and will provide patient education and advocacy to assist patients with accessing services of FHCW and make and follow-up on referrals for external services as part of the population health team.

Program Specific Essential Duties And Responsibilities

• Supports the care team with patient follow-up, reminders, and addresses barriers to care.
• Document all services as required and will assist in meeting all program requirements.
• Collects and analyzes data related to the project and works with data management staff for data collection.
• Provides outreach and meets with patients to explain services and coordinates the treatment plan.
• Facilitates referral process for patients using Healthify, E-referral, Community Help, and other tools as applicable.
• Advocates for patients regarding transportation, interpretation, health insurance, and FHCW services.
• Strong organizational and communication skills to act as a hub of information across multiple departments.
• Any and all other duties as assigned.

Qualifications And Education Requirements
