Presbyterian is seeking a Community Health Worker for the Lincoln County area in Southern NM to provide patient support in the ambulatory care setting according to established standards of care, in collaboration with Care Managers, Medical Providers, and other members of the healthcare team.

Qualifications

  • High school diploma or GED plus specialized, short-term training in clinical, medical skills.
  • Must be able to speak the language of the population served.

Responsibilities

  • Participate in planning and implementing the plan of care as developed by the Care Manager.
  • Report changes in patient condition as reported by the patient to the Care Manager and/or Primary Care Provider as appropriate. Support patient and family teaching.
  • Maintain a safe environment for patients, families, and visitors.
  • Keep current with community health care worker standards, PHS philosophy, and programs to maintain quality.
  • Maintain educational development through participation in education programs, current literature, in-service meetings, and workshops.
  • Perform other functions as required.
  • Interact effectively with customers, providing instructions and information, both written and verbal, as appropriate.
  • Participate in local, regional, or state community health care worker events and meetings.
