Community Health Worker - Lawrence County

CareStar, Inc. is currently seeking a Community Health Worker in the Clinical department to conduct program assessments using program and CareStar identified tools and process. The Community Health Worker is a home-based position that works independently to manage a caseload as needed.

THE POSITION

CareStar is looking for motivated individuals who:

• Bachelor's Degree
• 12 months experience in home and community-based services within the past 5 years.
• Knowledge of Medicare, Medicaid, home health care and Home and Community-Based waiver programs desirable.
• Adheres to the CareStar Rule in performance of job responsibilities.
• Understands and complies with CareStar Policies and Procedures.
• Maintains confidentiality as related to patient information. Any disclosures of confidential information made unlawfully outside the proper course of duty will be treated as a serious disciplinary offense. Follows the Acceptable Use Policy while using any information systems owned or controlled by CareStar, Inc.

This position is open in Lawrence County and surrounding counties.
