Community Health Worker- Trumbull County

Ready to join a growing company whose work profoundly impacts people’s lives in their community? CareStar, Inc. is currently seeking a Community Health Worker in the Clinical department to conduct program assessments using program and CareStar identified tools and processes. The Community Health Worker is a home-based position that works independently to manage a caseload as needed.

THE POSITION

CareStar is looking for motivated individuals who:

• Hold a Bachelor's Degree.
• Have 12 months experience in home and community-based services within the past 5 years.
• Possess knowledge of Medicare, Medicaid, home health care, and Home and Community-Based waiver programs (desirable).
• Adhere to the CareStar Rule in performance of job responsibilities.
• Understand and comply with CareStar Policies and Procedures.
• Maintain confidentiality related to patient information. Any unlawful disclosures of confidential information will be treated as a serious disciplinary offense. Follow the Acceptable Use Policy while using any information systems owned or controlled by CareStar, Inc.

This position is open in Trumbull County and surrounding counties.
