Job summary

Mercy Medical Center, part of Trinity Health Of New England, is hiring a Community Health Worker for a grant-funded role. The CHW will help homeless individuals access social services and healthcare, coordinating referrals and providing transportation as needed.

Qualifications include a high school diploma or notional equivalence, BLS, a valid driver’s license, and bilingual Spanish/English preferred. The position offers daytime hours with opportunities to adjust as population needs require.

Qualifications

• CHW Level I- Minimum high school diploma or equivalent.
• CHW Level II- Minimum Bachelor's Degree in human services or related field.
• Basic Life Support (BLS) certification.
• Current Driver's License in good standing.
• Bilingual (Spanish/English) strongly preferred.

Responsibilities

• Improve access to health and human services for homeless population through in-person interactions, telephone contacts, and coordination of referrals.
• Facilitate or conduct enrollment in available federal, state and local programs to increase access to health care and support services.
• Maintain frequent contact with community agencies serving vulnerable homeless and at-risk of homelessness populations.
• Provide individual & community support. Serves as a community liaison between target population and agencies and providers.
• Provide transportation assistance necessary to ensure the success of the populations served.

Position Purpose

T his is a grant funded position . The Talent Acquisition Partner will be able to provide more details in the form of a Grant Funded Position Agreement . In this grant the Community Health Worker assists homeless individuals in the local community gain access to key social service and healthcare providers/services to better their health and improve their lives.

What you will do
