Job summary

Mercy Medical Center in Western Massachusetts is seeking a Community Health Worker to join the grant-funded outreach team. The role focuses on connecting homeless individuals with health and social services and coordinating referrals to improve health outcomes.

Responsibilities include assisting enrollment in federal, state, and local programs, maintaining contact with community agencies, and providing transportation support.

Qualifications

• CHW Level I- Minimum high school diploma or equivalent
• CHW Level II- Minimum Bachelor's Degree in human services or related field
• Basic Life Support (BLS) certification
• Current Driver's License in good standing
• BILINGUAL (SPANISH/ENGLISH) STRONGLY PREFERRED

Position Purpose

T his is a grant funded position . The Talent Acquisition Partner will be able to provide more details in the form of a Grant Funded Position Agreement . In this grant the Community Health Worker assists homeless individuals in the local community gain access to key social service and healthcare providers/services to better their health and improve their lives.

What you will do
