Job summary

Your Health Organization is actively seeking a dedicated Community Health Worker to serve patients in the Athens, GA area. This role involves home and facility visits, delivering medications and DME, and coordinating with care teams to improve access to care.

The CHW will travel daily, work full-time, and follow a 8:00AM-5:00PM schedule with M-F shifts, supporting care in homes, ALFs, and clinics through in-person and telehealth interactions.

Qualifications

• Certified medical professional with current license.
• Phlebotomy experience preferred; willing to train if not.
• Public health or home health experience preferred.
• Experience with DME delivery or logistics preferred.
• Proficient with technology for RPM, telemedicine, and EMR.
• Strong organizational and communication skills.
• Must have reliable transportation and valid driver’s license.
• Ability to travel between offices and facilities.

Responsibilities

• Coordinate with care team to ensure patient care follows the care plan.
• Communicate via phone, email, Teams, and in person.
• Facilitate visits to ALFs and homes to support patient care.
• Deliver medications; review instructions and reconcile medications.
• Coordinate telehealth between patients and pharmacists/providers.
• Set up RPM equipment training and provide ongoing support.
• Deliver DME to homes/ALFs; train patients on proper use.
• Perform venipuncture for lab testing and proper specimen handling.
• Collect blood specimens during visits to support care.
