Job summary

Your Health in Marietta, GA is seeking a dedicated Community Health Worker to serve patients in the Marietta area. The CHW will visit patients at homes and ALFs, facilitate visits with providers, deliver medications and DME, and coordinate telehealth and RPM setup.

A full-time role requiring daily travel with 12-hour shifts (7:00AM-7:00PM). The team serves GA and SC with patient-centered care and access to services.

Qualifications

• Certified Medical Assistant, Paramedic, or Licensed Nurse preferred; licenses in good standing.
• Phlebotomy experience preferred; willing to train if not experienced.
• Public health or home health experience preferred.
• Experience with DME delivery or logistics operations preferred.
• Proficiency with technology tools for remote monitoring, telemedicine, and EMR.
• Strong organizational and time management skills.
• Ability to read and communicate effectively; strong written and verbal communication.
• Must be able to travel between offices and facilities.

Responsibilities

• Work closely with care team members to ensure coordinated patient care and adherence to the care plan.
• Communicate effectively via phone, email, and in-person.
• Facilitate and coordinate visits with PTs, OTs, and Providers in homes and ALFs.
• Deliver medications to patients' homes and perform medication reconciliation.
• Coordinate telehealth visits between patients and pharmacists and providers.
• Set up training and instruct patients on RPM equipment and provide ongoing support.
• Deliver DME to patients in homes and ALFs and provide caregiver training.
• Perform routine venipuncture for lab testing and collect blood samples during visits.
