Job summary

The Fund for Public Health in New York City is seeking a Community Health Worker to support the BOLD program and neighborhood health initiatives across all five boroughs. The role focuses on brain health, dementia risk reduction, and equitable access to resources for older adults and caregivers.

Based in NYC with hybrid office/remote work, the CHW will engage communities, conduct outreach, deliver education, and assist with data collection and program administration.

Qualifications

• Fluency in English is required.
• Spanish or Mandarin a plus for community engagement.
• Availability to work evenings and weekends.

Responsibilities

• Assist with planning and executing health awareness and promotion activities (evenings/weekends possible).
• Deliver presentations and workshops to diverse audiences.
• Participate in community outreach events and health fairs.
• Input outreach activities into databases and documents.
• Develop relationships with community residents and stakeholders.
• Support data collection and reporting of program outcomes.
• Provide clerical support and assist with program administration.
