Working closely with primary care and behavioral health providers, they ensure our patients’ priorities are heard and addressed. General Requirements:

• High School Diploma or GED
• 2 years’ experience in health – or service – related field
• Valid driver’s license and clean driving record
• Bilingual in English & Spanish
• Knowledge of community resources in the Columbia River Gorge, especially those available to older adults
