Job summary

Choptank Community Health is seeking a Community Health Worker (CHW – CRC) to bridge clinicians, community services, and patients, focusing on increasing colorectal cancer screening rates among our patient population.

This is a grant-funded, full-time position reporting to the Director of Quality & Population Health. The CHW will conduct targeted outreach, education, reminders, and navigation to support screening workflows across clinic sites on the Eastern Shore.

Qualifications

• High school diploma or equivalent.
• CHW background preferred; certification required after starting.
• Strong communication and problem-solving skills.
• Knowledge of medical terminology and referrals.
• Basic computer skills (Word, Excel, Outlook).
• Ability to travel to CCHS sites weekly.

Responsibilities

• Provide targeted outreach to patients overdue for colorectal cancer screening.
• Educate patients on screening options using culturally appropriate approaches.
• Conduct patient reminders (phone/text/mail/in-person) to support screening completion.
• Assist patients with scheduling screening and addressing barriers (transport, health literacy).
• Provide patient navigation across the screening continuum and follow-up per clinical staff.
• Collaborate with clinic teams to ensure proper screening workflow.
• Support documentation, tracking and data capture of screening activities.
• Participate in team meetings and quality improvement efforts.
