The Community Health Worker (CHW – CRC) at Choptank Community Health System is a bridge between healthcare providers, community services and patients to improve colorectal cancer screening rates. The role uses culturally responsive, patient-centered strategies to educate and navigate patients, overcome barriers, and support screening completion through reminders, education, and care coordination.

Qualifications

• High school diploma or equivalent required.
• CHW or Social Work background preferred.
• CHW certification required after starting employment.

Responsibilities

• Provide targeted outreach to patients overdue or at risk for CRC screening.
• Educate patients on CRC prevention and screening using culturally appropriate approaches.
• Conduct patient reminders via phone, text, mail, or in person.
• Assist scheduling and address barriers such as transportation and health literacy.
• Provide patient navigation across screening, including follow-up for abnormal results.
• Document, track, and report data related to screening activities.
