Job summary

Bay Area Community Health in Fremont, CA seeks a full-time Community Health Worker III to advance access to resources and coordinate training for staff and partners. You will travel to clinics, schools, homes and other sites, supporting clients and improving outcomes in a mission-driven environment.

The role emphasizes HIPAA compliance and ongoing professional development. A 2+ year background in community-based care, CHW and CEC certifications, and a strong bilingual ability are valued.

COMMUNITY HEALTH WORKER - CHW III QUALIFICATIONS

The ideal fit is a compassionate, mission-driven person who wants to go above and beyond for the populations we serve. We would prefer someone with 2+ years of community-based healthcare service experience, an AA/AS in a field related to health, and bilingual communication skills. What matters most is that you have:

• Current CHW certification and CEC certification (if applicable)
• Ability to complete timely recertification and retraining processes
• Ability to travel to various BACH locations, such as clinics, schools, homes, warming centers, homeless encampments, and patient appointments, and work outside in various weather
• Ability to complete physical tasks like using a computer/keyboard, moving up to 50 lbs., and other duties typical of an office environment
• High school diploma or GED

A DAY IN THE LIFE

Standard work hours are Monday - Friday, 8:00 am - 5:00 pm.

Our Community Health Worker - CHW III supports our dedicated team in serving clients' needs by helping expand access to resources, developing a network of community partners, and training other CHWs on our processes and industry best practices. You'll coordinate internal, external, and online training for staff members and monitor their productivity to ensure everyone's doing the best they can for clients. Outside of internal development, you'll proactively research, contact, and strengthen connections with local healthcare providers, specialists, and grant-funded programs. By developing these partnerships, you'll help increase our clients' access to and knowledge of invaluable resources.

In all your work activities, you'll make sure to follow HIPAA regulations, comply with BACH guidelines, and support continuous improvement following the PDSA model. The Community Health Worker - CHW III also evaluates clients' living situations and performs thorough housing, crisis, intake, and SDOH assessments to properly identify needs and determine the appropriate steps to take. You'll respond to information requests, help address and resolve complex issues, and prepare sensitive/confidential correspondence when needed. Assisting in other areas as assigned, you'll help strengthen our team, broaden available support services, and make a lasting positive impact!
