Job summary

Bay Area Community Health in Fremont, CA is seeking a full-time Community Health Worker III to connect clients with care and resources, expand partner networks, and support staff development. You will train staff, conduct comprehensive need assessments, and ensure HIPAA compliance while driving process improvements using the PDSA model.

Your duties include evaluating home living situations, conducting housing and SDOH assessments, and linking individuals with the right services and resources.

Qualifications

• CHW certification with ability to complete recertification or retraining when needed
• CEC certification (if applicable)
• High school diploma or equivalent
• Ability to travel to different locations (clinics, schools, etc.) and work outdoors in varying conditions
• Ability to regularly move up to 50 lbs., use a computer and keyboard, and perform routine office activities

Responsibilities

• Train staff and conduct thorough need assessments to connect clients with resources and support
• Coordinate external/online educational opportunities for CHW staff and ensure HIPAA compliance
• Build relationships with healthcare providers, organizations, and grant-funded programs to expand the partner network
• Evaluate home living situations and perform housing, crisis, intake, and SDOH assessments
• Connect individuals with appropriate resources and social services, advocate for their well-being, and follow up to monitor care effectiveness
• Handle information requests, resolve complex problems, and prepare confidential correspondence
• Assist with inventory, staff productivity reviews, and other administrative duties

Pay: $27.08 - $31.39/hour

Schedule: Monday - Friday, 8:00 am - 5:00 pm, with some evenings or weekends as needed

COMMUNITY HEALTH WORKER III HIGHLIGHTS:

As a member of our compassionate, mission-driven team, you'll help elevate the levels of care we provide by effectively training staff and conducting thorough need assessments for connecting clients with the correct resources and support. You'll drive continual process improvements using the PDSA model and coordinate external/online educational opportunities for the other CHW staff while ensuring compliance with all HIPAA and BACH regulations. By building relationships with healthcare providers, specialists, community organizations, and grant-funded programs, you'll help expand our partner network and develop the level of support we can offer.

Your patient-facing responsibilities include evaluating home living situations, conducting housing, crisis, intake, and SDOH assessments, and connecting individuals with the appropriate resources, social services, or community partners. You'll keep patients engaged, advocate for their well-being, and follow up with them to monitor care effectiveness. When appropriate, you'll respond to requests for information, resolve complex problems, and prepare sensitive/confidential correspondence. Attentive to detail, you'll assist with administrative duties like maintaining inventory records, monitoring staff productivity, and reviewing files for compliance. The Community Health Worker III also helps with additional responsibilities as assigned. Your efforts will strengthen the team, expand our resource offerings, and improve how we serve patients!

What You Need:

• CHW certification with the ability to complete recertification or retraining processes as needed
• CEC certification (if applicable)
• High school diploma or equivalent
• Ability to travel to different locations (i.e., clinics, schools, homeless encampments, warming centers, patients' homes or appointments, etc.) and work outside in various temperatures/weather conditions
• Ability to regularly move up to 50 lbs., use a computer and keyboard, and perform physical duties typical of an office environment (such as reaching, bending, and walking/sitting/standing for prolonged periods)

Our ideal Community Health Worker III has at least 2 years of experience in a community-based healthcare service capacity. Bilingual communication skills and an AS/AA in health (or a related field) would also be preferred but aren't required for the right person.
