We are currently seeking a Community Health Worker in the Clinical department to conduct program assessments using established tools and processes. This is a home-based position that requires independent management of a caseload.

The Position

CareStar is looking for motivated individuals who:

• Have a Bachelor's Degree
• Possess 12 months of experience in home and community-based services within the past 5 years
• Have knowledge of Medicare, Medicaid, home health care, and Home and Community-Based waiver programs (desirable)
• Adhere to the CareStar Rules in performing job responsibilities
• Understand and comply with CareStar Policies and Procedures
• Maintain confidentiality related to patient information; unlawful disclosures are serious disciplinary offenses
• Follow the Acceptable Use Policy for all information systems owned or controlled by CareStar, Inc.

This position is open in Mahoning County and surrounding areas.
