We are currently seeking a Community Health Worker in the Clinical department to conduct program assessments using established tools and processes. This home-based position requires independent management of a caseload.

The Position

CareStar is looking for motivated individuals who:

• Hold a Bachelor's Degree
• Have 12 months of experience in home and community-based services within the past 5 years
• Possess knowledge of Medicare, Medicaid, home health care, and Home and Community-Based waiver programs (desirable)
• Adhere to the CareStar Rules in performing job responsibilities
• Understand and comply with CareStar Policies and Procedures
• Maintain confidentiality regarding patient information; unlawful disclosures outside the scope of duty will be treated seriously
• Follow the Acceptable Use Policy when using CareStar's information systems

This position is available in Summit County and surrounding areas.
