Community Health Worker - Summit County

We are currently seeking a Community Health Worker in the Clinical department to conduct program assessments using established tools and processes. This is a home-based position that requires independent caseload management.

The Position

CareStar is looking for motivated individuals who:

• Hold a Bachelor's Degree
• Have 12 months of experience in home and community-based services within the past 5 years
• Possess knowledge of Medicare, Medicaid, home health care, and Home and Community-Based waiver programs (desirable)
• Adhere to the CareStar Rules in job performance
• Understand and comply with CareStar Policies and Procedures
• Maintain confidentiality of patient information; unlawful disclosures are serious disciplinary offenses
• Follow the Acceptable Use Policy regarding CareStar's information systems
