Community Health Worker - Trumbull County

We are currently seeking a Community Health Worker in the Clinical department to conduct program assessments using approved tools and processes. This is a home-based position that requires independent management of a caseload.

The Position

CareStar is looking for motivated individuals who meet the following criteria:

• Bachelor's Degree
• At least 12 months of experience in home and community-based services within the past 5 years
• Knowledge of Medicare, Medicaid, home health care, and Home and Community-Based waiver programs is desirable
• Adherence to the CareStar Rules in performance of responsibilities
• Understanding and compliance with CareStar Policies and Procedures
• Maintaining confidentiality related to patient information; unlawful disclosures outside the proper course of duty will be treated as a serious disciplinary offense
• Following the Acceptable Use Policy when using CareStar's information systems
