Job summary

Reid Health is seeking a Community Health Worker to support outreach, education, and resource navigation across our rural health initiatives. Qualifications

• Education Required: High school diploma or equivalent

• Experience Required: Minimum of 2 years’ experience with a hospital, health agency, or community service organization with demonstrated communication and interpersonal skills with the ability to engage effectively with individuals from diverse cultural and socioeconomic backgrounds.
• Experience Preferred: Knowledge and experien Licensure
• Licensure: Certified Community Health Worker (Indiana) within 60 days of hire.
• Food Safety Certification required or successful completion within 6 months of hire to support food access programming.

Responsibilities

• Community Health Education & Outreach: Develop, deliver, and evaluate health education programs and initiatives aligned with Reid Health’s Community Health Needs Assessment priorities, Indiana Department of Health priorities, and emerging community needs.
• Resource Navigation & Patient Support: Establish trusting relationships with patients, families, community members, and partner organizations while providing support, encouragement, and advocacy.
• Transportation Access Coordination: Review transportation requests, collect required information, identify appropriate transportation options, schedule services, and document arrangements.
• Food Access Support: Connect individuals to food resources including food pantries, community meals, mobile food distributions, grocery support programs, SNAP, WIC, and nutrition education services.
• Community Partnerships & Regional Collaboration: Serve as an active participant in the Region 4 Community Health Worker Network by sharing resources, strengthening referral pathways, identifying service gaps, and supporting coordinated resource navigation efforts.
• Professional Development & Compliance: Maintain Community Health Worker certification and required continuing education units (CEUs).
• Community Screenings (as applicable): Conduct blood pressure and glucose screenings at community events when qualified and trained to do so.

Community Health Worker 9325 Integrated Community Access and Chronic Disease Network (CDMP) This grant‑funded position is authorized in one‑year funding periods and is contingent upon continued grant funding, program requirements, and achievement of required outcomes. Grant funding may be renewed for additional periods; however, if funding is not renewed or otherwise becomes unavailable, employment in this position will end at the conclusion of the applicable funding period. Employment with Reid Health is at will, meaning that either the employee or Reid Health may end the employment relationship at any time, with or without notice and with or without cause, subject to applicable law. This Rural Health Transformation Program is supported by the Centers for Medicare & Medicaid Services (CMS) of the U.S. Department of Health and Human Services (HHS) as part of a financial assistance award totaling $206,927,896.80 with 100 percent funded by CMS/HHS. The contents are those of the author(s) and do not necessarily represent the official views of, nor an endorsement, by CMS/HHS, or the U.S. Government Schedule: Monday through Friday 8:00 a.m.–5:00 p.m.; however, schedules may vary based on community events and departmental needs. Evening and weekend work will be required as needed.

About the Position

The Community Health Worker assists Reid Health in achieving its mission to lead our communities to well‑being, one person at a time, by effectively developing, coordinating, and implementing activities and outreach initiatives that align with Reid Health’s Community Health Needs Assessment, emerging and urgent community needs and/or the Indiana Department of Health’s public health priorities. The Community Health Worker is primarily responsible for providing support to the community through outreach, education, and resource navigation for patients and individuals in Reid’s service area. This includes helping address non‑medical drivers of health such as food access, transportation support, and linkage to vital community resources. This position requires a commitment to the overall improvement of community health, with a focus on the prioritized health needs for Reid Health and the state of Indiana. The Community Health Worker is also expected to build and maintain strong partnerships within Reid’s nonprofit network to support coordinated referrals, improve access to services, and strengthen community‑based support for individuals and families.

Overview of Responsibilities

• Community Health Education & Outreach
  • Develop, deliver, and evaluate health education programs and initiatives aligned with Reid Health’s Community Health Needs Assessment priorities, Indiana Department of Health priorities, and emerging community needs.
  • Promote health literacy by providing clear, accurate, and accessible health information that supports informed decision‑making and healthy behaviors.
  • Collaborate with healthcare providers, schools, faith‑based organizations, businesses, government agencies, and community partners to improve community health outcomes and expand access to services.
  • Plan, coordinate, and participate in community outreach events and health fairs, including setting up and managing vendor tables, engaging attendees, sharing health and community resources, gathering feedback, and supporting efforts to increase awareness of organizational programs and services.
• Resource Navigation & Patient Support
  • Establish trusting relationships with patients, families, community members, and partner organizations while providing support, encouragement, and advocacy.
  • Connect individuals to healthcare, social services, community resources, support programs, and other services that address health‑related social needs.
  • Assist individuals in navigating healthcare systems, accessing services, overcoming barriers to care, and following through with referrals.
  • Identify emerging community needs and service gaps and communicate trends to support program development and community health planning.
• Transportation Access Coordination
  • Review transportation requests, collect required information, identify appropriate transportation options, schedule services, and document arrangements.
  • Confirm transportation details, communicate expectations to individuals and providers, and coordinate changes or issue resolution as needed.
  • Conduct follow‑up activities related to scheduled transportation, missed rides, cancellations, and service concerns.
• Food Access Support
  • Connect individuals to food resources including food pantries, community meals, mobile food distributions, grocery support programs, SNAP, WIC, and nutrition education services.
  • Support enrollment, ordering, scheduling, communication, transportation coordination, and follow‑up activities related to food access programs.
  • Participate in and support food distribution events to ensure services are organized, welcoming, safe, and delivered with dignity.
• Community Partnerships & Regional Collaboration
  • Serve as an active participant in the Region 4 Community Health Worker Network by sharing resources, strengthening referral pathways, identifying service gaps, and supporting coordinated resource navigation efforts.
  • Attend community meetings, coalitions, workshops, and partner convenings to expand resource knowledge, strengthen collaborative relationships, and improve service coordination.
  • Build and maintain effective working relationships with community organizations, healthcare partners, and regional stakeholders.
• Professional Development & Compliance
  • Maintain Community Health Worker certification and required continuing education units (CEUs).
  • Participate in ongoing professional development activities and remain current on best practices, evidence‑based interventions, non‑medical drivers of health, patient advocacy, rural health priorities, and community health trends.
  • Complete required documentation, reporting, and program records in accordance with organizational policies and grant requirements.
• Community Screenings (as applicable)
  • Conduct blood pressure and glucose screenings at community events when qualified and trained to do so.
  • Follow established community screening policies and procedures, maintain required equipment and supplies, and complete required training and competency requirements.

Education & Experience
