Job summary

Northern Light Health is seeking a community health worker to help patients overcome barriers, access resources, and take meaningful steps toward better health and well-being.

You’ll build trusted relationships with patients in their homes, healthcare settings, and communities while helping them: We’re looking for an empathetic listener, strong communicator, and natural relationship-builder who is comfortable with outreach and home visits.

Qualifications

• Experience working with patients to connect to resources and support systems.
• Strong verbal and written communication skills for diverse communities.
• Ability to build trust and rapport in home and community settings.

Responsibilities

• Connect patients with healthcare and community resources.
• Help patients set and work toward health goals.
• Assist with accessing benefits, food assistance, housing resources, prescriptions, and other support.
• Navigate appointments and the healthcare system for patients.
• Address social challenges that may impact health.

• Address social challenges that may impact their health
