Job summary

MaineHealth Behavioral Health in North Conway, NH is seeking a Community Health Worker to help patients access local resources, support disease self-management, and coordinate culturally appropriate care. The role is full-time with day shifts and a focus on community-based service delivery.

The candidate should be a resident of the community with knowledge of local resources; education of a high school diploma or GED is preferred, and basic computer skills are desirable.

Summary:
The Community Health Worker role assists patients in accessing resources/services, supports disease self-management, and coordinates culturally-appropriate care and services.

Required Minimum Knowledge, Skills, and Abilities (KSAs)
  • Education: High school diploma or GED preferred.
  • License/Certifications: If driving required for role, Valid Driver’s license, clean driving record, and personal vehicle for use required.
  • Experience: Two years of experience working in a social service or health care setting or equivalent educational setting preferred.
  • Additional Skills/Requirements Required: Must be a resident of the community being served with good knowledge of the resources of their community.
  • Additional Skills/Requirements Preferred: Basic computer skills preferred.

The pay range for this position is $20.68 to $25.10 per hour.

Compensation for part-time positions is determined on a prorated basis consistent with the scheduled hours and applicable full-time equivalent (FTE).
