MaineHealth is seeking a Community Health Worker to join the Lend a Hand team within our care management program. You will help patients overcome barriers, access services, and progress toward health goals in a patient-centered, community-focused setting.

The role is full-time, 40 hours/week with a day shift and a hybrid arrangement (up to 2 days/week remote after training). Driving may be required; use your vehicle for work as needed.

Qualifications

  • High school diploma or GED preferred.
  • Must be resident of the community served with good knowledge of local resources.
  • Basic computer skills preferred.

  • Support disease self-management and coordinate culturally appropriate care.

We have an exciting opportunity for a Community Health Worker (CHW) to join Lend a Hand, part of our growing care management program! Our CHWs provide culturally appropriate complex care management in the primary care setting. We work with each patient for approximately 6 months, helping them to overcome barriers, navigate the healthcare system, and make meaningful progress toward health-related goals. If you are passionate about improving health outcomes and working as part of a patient-centered team, consider joining us! 40 Hours/week, Full-time, Day shift, Hybrid (up to 2 days/week remote after training is completed). Health care management experience would be advantageous for success in this role.

Summary

The Community Health Worker role assists patients in accessing resources/services, supports disease self-management, and coordinates culturally appropriate care and services.

Required Minimum Knowledge, Skills, And Abilities (KSAs)
  • Education: High school diploma or GED preferred.
  • License/Certifications: If driving required for role, Valid Driver’s license, clean driving record, and personal vehicle for use required.
  • Experience: Two years of experience working in a social service or health care setting or equivalent educational setting preferred.
  • Additional Skills/Requirements Required: Must be a resident of the community being served with good knowledge of the resources of their community.
  • Additional Skills/Requirements Preferred: Basic computer skills preferred.

The pay range for this position is $20.68to $25.10 per hour.

Compensation for part-time positions isdeterminedon a prorated basis consistent with the scheduled hours and applicable full-time equivalent (FTE).
