Responsibilities

This position supports the MaineMOM Program and works with pregnant and postpartum individuals receiving treatment for substance use disorder. The Community Health Worker (CHW) serves as a key member of the care team, providing patient outreach, care coordination, navigation, advocacy, and support to help participants engage in treatment and access needed services.

Responsibilities include building trusting relationships with patients, addressing barriers to care, assisting with appointment scheduling and follow-up, facilitating referrals, and connecting patients to community resources that support recovery, maternal health, parenting, housing, transportation, food security, and other social needs. The CHW may collaborate with recovery residences, supervised withdrawal facilities, behavioral health providers, primary care practices, social service agencies, and other community partners to ensure patients receive comprehensive support.

The ideal candidate is passionate about supporting individuals and families affected by substance use disorder, demonstrates strong communication and organizational skills, and is committed to fostering a welcoming, nonjudgmental, and trauma-informed environment for patients throughout pregnancy and the postpartum period.

Summary:
The Community Health Worker role assists patients in accessing resources/services, supports disease self-management, and coordinates culturally-appropriate care and services.

Required Minimum Knowledge, Skills, and Abilities (KSAs)

1. Education: High school diploma or GED preferred.
2. License/Certifications: If driving required for role, Valid Driver's license, clean driving record, and personal vehicle for use required.
3. Experience: Two years of experience working in a social service or health care setting or equivalent educational setting preferred.
4. Additional Skills/Requirements Required: Must be a resident of the community being served with good knowledge of the resources of their community.
5. Additional Skills/Requirements Preferred: Basic computer skills preferred.

The pay range for this position is $20.68to $25.10 per hour.

Compensation for part-time positions isdeterminedon a prorated basis consistent with the scheduled hours and applicable full-time equivalent (FTE).
