Job summary

MaineHealth's Pen Bay Hospital is seeking a Community Health Worker to help patients access resources, support disease self-management, and coordinate culturally appropriate care. The role primarily focuses on youth, families, and pregnant/postpartum individuals within the community, connecting clients to services and insurance options.

Responsibilities include outreach, maintaining records, and partnering with organizations to improve access to care.

Qualifications

• Education: High school diploma or GED preferred.
• License/Certifications: Valid driver's license, clean driving record, and personal vehicle for use required."
• Experience: Two years of experience working in a social service or health care setting or equivalent educational setting preferred.
• Must be a resident of the community being served with good knowledge of the resources of their community.
• Basic computer skills preferred.

The Community Health Worker role assists patients in accessing resources/services, supports disease self-management, and coordinates culturally appropriate care and services.

This position focuses primarily on supporting youth and their families as well as pregnant and postpartum individuals within the community. Responsibilities include connecting clients to treatment services, resources, assisting with insurance enrollment, maintaining accurate records data, conducting outreach to strengthen partnerships, and connecting clients to resources.

Required Minimum Knowledge, Skills, and Abilities (KSAs)

1. Education: High school diploma or GED preferred.
2. License/Certifications: If driving required for role, Valid Driver's license, clean driving record, and personal vehicle for use required.
3. Experience: Two years of experience working in a social service or health care setting or equivalent educational setting preferred.
4. Additional Skills/Requirements Required: Must be a resident of the community being served with good knowledge of the resources of their community.

The pay range for this position is $20.68to $25.10 per hour.

Compensation for part-time positions isdeterminedon a prorated basis consistent with the scheduled hours and applicable full-time equivalent (FTE).
