Qualifications

Responsibilities

  • Care Coordination & Patient Support bridging patients, providers, and community resources.
  • Assist navigation of healthcare, behavioral health, and social services systems.
  • Identify barriers to care and develop solutions with care teams.
  • Conduct follow-up outreach via phone, visits, and home visits as needed.
  • Maintain documentation and data entry per policy and participate in projects.
