Clarity Healthcare is seeking a compassionate Community Health Worker to help individuals overcome barriers to care and improve health and well-being. You will act as a liaison between patients, providers, and community resources to navigate complex systems and access essential services.

Qualifications

• High school diploma or equivalent required.

Responsibilities

• Care Coordination & Patient Support bridging patients, providers, and community resources.
• Assist navigation of healthcare, behavioral health, and social services systems.
• Identify barriers to care and develop solutions with care teams.
• Conduct follow-up outreach via phone, visits, and home visits as needed.
• Maintain documentation and data entry per policy and participate in projects.
