for individuals by connecting them with healthcare providers, behavioral health services, and community resources.· Build meaningful relationships with patients and empower them to achieve their health and wellness goals.· Educate individuals and families on healthcare resources, preventative care, wellness initiatives, and available support programs.· Conduct outreach activities through phone calls, community visits, home visits, and community events to engage individuals in care and services.· Identify barriers to care and collaborate with patients and interdisciplinary teams to develop effective solutions.· Advocate for patients while promoting culturally responsive, person-centered care.· Assist individuals with understanding insurance coverage options and completing eligibility and enrollment applications, including Qualified Health Plans (QHPs).· Collaborate with healthcare providers, care teams, and community partners to improve care coordination and health outcomes.· Maintain accurate documentation, records, and data entry in accordance with organizational requirements.· Support departmental initiatives, quality improvement efforts, and other assigned projects.Education, Experience, and/or Credential Qualifications:· High School Diploma or equivalent required.· Bachelor's degree in Health Sciences, Social Work, Human Services, Public Health, or a related field preferred.· Two (2) years of experience in community health, patient navigation, healthcare outreach, human services, case management, or a related field preferred.· Knowledge of community health principles, social determinants of health, harm reduction practices, and motivational interviewing techniques.· Knowledge of local community resources, support programs, and social service systems.· Strong customer service, relationship-building, communication, and problem-solving skills.· Proficiency with Microsoft Office applications including Excel, Outlook, Teams, and Word.· Ability to work independently while collaborating effectively within a multidisciplinary team.Additional Qualifications:· Valid driver's license, reliable transportation, and current auto insurance required.· Excellent interpersonal, active listening, and communication skills.· Strong organizational skills with attention to detail.· Ability to communicate effectively with diverse populations in a clear, compassionate, and professional manner.· Experience in customer service, peer support, coaching, community outreach and engagement, healthcare or behavioral health settings, public speaking, and health education is beneficial.· Complete an approved Community Health Worker training program through a recognized curriculum provider.· Must complete required training related to Trauma-Informed Care, Behavioral Health Conditions in Children, Suicide Prevention, and Substance Use Disorders.· Must attain First Aid Certification within three (3) months of hire and maintain certification thereafter.· Must attain CPR Certification within three (3) months of hire and maintain certification thereafter.· Tuberculosis (TB) screening required for all direct care staff.Physical Requirements· Perform sedentary work requiring exertion of up to 10 pounds of force occasionally and negligible force frequently· Sit for extended periods while providing services, completing documentation, and attending meetings· Occasionally walk and stand while conducting client services and participating in program activities· Perform repetitive hand, finger, and arm movements associated with typing, writing, and electronic documentation· Lift, carry, push, or pull light objects as necessary to perform essential job functions· Utilize computers, office equipment, and electronic health record
