Job summary

MaineHealth is seeking a Community Health Worker (CHW) to join Lend a Hand, part of our care management program. CHWs provide culturally appropriate, complex care management in primary care settings and work with each patient for about 6 months to overcome barriers and improve health outcomes.

Position is 40 hours/week, full-time, day shift, hybrid (up to 2 days/week remote after training). We offer paid parental leave, flexible work options, and professional development to support your career

Qualifications

  • Education: High school diploma or GED preferred.
  • License/Certifications: If driving required for role, Valid Driver's license, clean driving record, and personal vehicle for use required.
  • Experience: Two years of experience working in a social service or health care setting or equivalent educational setting preferred.
  • Additional Skills/Requirements Required: Must be a resident of the community being served with good knowledge of the resources of their community.
  • Additional Skills/Requirements Preferred: Basic computer skills preferred.

We have an exciting opportunity for a Community Health Worker (CHW) to join Lend a Hand, part of our growing care management program! Our CHWs provide culturally appropriate complex care management in the primary care setting. We work with each patient for approximately 6 months, helping them to overcome barriers, navigate the healthcare system, and make meaningful progress toward health-related goals. Required Minimum Knowledge, Skills, and Abilities (KSAs)

The pay range for this position is $20.68to $25.10 per hour.

Compensation for part-time positions isdeterminedon a prorated basis consistent with the scheduled hours and applicable full-time equivalent (FTE).
