Community Health Worker

Community Health Workers (CHWs) assess and address clients' social needs by developing actionable plans to connect individuals with resources that address Social Drivers of Health (SDOH). CHWs hold an essential role in advancing community health by addressing systemic barriers and empowering individuals to improve their quality of life.

Under indirect supervision, CHWs coordinate with medical providers, primary care teams, social services, and community organizations to deliver short-term care coordination, educational support, and resource navigation. CHWs may also provide peer support in areas such as mental health, substance use recovery, housing instability, food insecurity, and other social encounters. This role offers comprehensive training to obtain CHW certification, supporting skill development, and professional growth. CHWs provide direct support in clinical, community, and home settings, focusing on improving clients' overall health, well-being, and access to services.

Conditions of Employment
  • Possession of a valid New Mexico driver's license is a requirement for this job.
  • Must pass a pre-employment criminal background check.
  • Fingerprinting, and subsequent clearance, may be required for this position depending on assignment.
  • May be required to obtain a Community Support Worker (CSW) certification within six months of hire.
  • Employees who provide services or work in patient care or clinical areas are required to be in compliance with the University's influenza vaccination requirement.
  • Successful candidate must submit to post-offer, pre-employment physical examination and medical history check.
Minimum Qualifications

Preferred Qualifications
  • This role supports older adult populations, candidates 50 years and older are preferred
  • Must be willing to travel using a personal vehicle.
  • Well-versed in computers and technology, Bilingual.
  • Existing certification as a Community Health Worker (CHW) is a plus; training and certification will be provided for eligible candidates.
