Bronx Community Health Network (BCHN) is seeking a Community Health Worker to engage medically underserved populations in the Bronx. The CHW will promote health, assist with care navigation, and connect patients to needed services within the Population Health team.

The ideal candidate has CHW experience, strong communication, bilingual English/Spanish, and a commitment to equitable access to care. This full-time role offers steady weekday hours and occasional weekend coverage as needed.

Qualifications

  • High school diploma or equivalent required; Associate degree preferred.
  • 2+ years of CHW experience or comparable education/training experience preferred
  • CHW Certificate (Preferred)
  • Lived experience, cultural knowledge, or community familiarity is strongly valued.
  • Experience in community outreach, case management, health education, or social services preferred.
  • Bilingual English and Spanish.

Responsibilities

  • Conduct outreach to patients in clinic, community, home, or by phone.
  • Assist patients in scheduling medical and preventive appointments.
  • Provide follow-up outreach for chronic disease management.
  • Help patients understand care plans and follow provider instructions.
  • Assist with referrals to housing, food, mental health, transportation, and social services.
  • Support patients navigating health insurance enrollment and benefits.
  • Deliver basic health education on prevention and healthy lifestyles.

Position Title: Community Health Worker (CHW)

Supervisor: Asst. Director of Population Health

Job Type: Full-Time (37.5 hours weekly)

Department: Population Health

Schedule Shift: Monday- Friday 9:00am-5:30pm and weekends as needed.

Bronx Community Health Network (BCHN): was founded in 1996 to ensure that the socio-economically depressed and medically underserved residents of the Bronx have access to high-quality and comprehensive healthcare services, Bronx Community Health Network (BCHN) is a not-for-profit, community-based organization and Federally Qualified Health Center. The organization provides a wide range of comprehensive, coordinated primary medical, oral, and mental health care and related diagnostic, pharmacy, social support, and enabling services for medically underserved/uninsured residents. BCHN's service area includes the entire borough of the Bronx, including all 25 zip codes.

BCHN mission: To improve the health of people and families in the Bronx; provide access to affordable, quality health care, especially for uninsured persons; promote disease prevention, early treatment and healthful lifestyles; and obtain financial and other resources for programs and services.

About the Role: The Community Health Workers serves as a trusted member of the community and a key part of the care team. The CHW promotes health, encourages healthy behaviors, supports patients in accessing services, and reduces barriers to care. The CHW works closely with clinical providers, care coordinators, and external partners to improve health outcomes, address social determinants of health, and support patients in navigating the healthcare system. The CHW reports directly to the CHW Supervisor.

  • Conduct outreach to patients in the clinic, community, home settings, or by phone to promote engagement in care.
  • Assist patients in scheduling medical, behavioral health, and preventive appointments.
  • Provide follow-up outreach related to chronic disease management, preventive care, and care-plan adherence.
  • Help patients understand their care plans and support them in following provider instructions.
  • Assist with referrals to community services, including housing, food assistance, mental health resources, transportation, and social support.
  • Support patients in navigating health insurance enrollment, benefits, and health system processes.
  • Deliver basic health education on prevention, wellness, chronic disease management, and healthy lifestyle practices.
  • Provide culturally responsive education that reflects the needs of the population.
  • Reinforce provider-directed education using appropriate teaching tools and health literacy methods.
  • Document patient interactions, referrals, and outreach efforts in the electronic health record (EHR) and other data-tracking systems.
  • Maintain accurate, timely, and confidential records in compliance with HIPPA and organizational policy.
  • Assist in reporting outcomes and participating in quality improvement activities.
  • Participate in community events, health fairs, and outreach campaigns.
  • Build relationships with local organizations, agencies, and social service providers.
  • Represent the organization in community partnerships and coalitions as assigned.
  • Work with care teams, population health staff, and behavioral health providers to address patient needs.
  • Attend team meetings, conferences, and training sessions.
  • Communicate patient barriers or concerns to the appropriate clinical or supervisory staff.
  • Maintains knowledge of and compliance with FQHC, HRSA, Article 28, HIPPA, OSHA, infection control, patient privacy, and applicable federal, state, and local regulatory requirements. Supports audit readiness, survey preparation, documentation standards, and organizational policies as applicable to the role.
  • Performs other duties as assigned based on organizational needs, including support for new site openings, mobile health operations, emergency preparedness, and special projects.
Qualifications and Skills

  • Ability to work with diverse populations using culturally competent and trauma-informed approaches.
  • Basic computer literacy, including the ability to use EHR systems and documentation tools.
  • Ability to work independently and as part of an interdisciplinary team.
  • Ability to maintain effective professional and trusting relationships with patients, families, community partners, and clinical teams.
Salary Range: The base salary range for this position $55,000 - $65,000 annually.

The total compensation for the finalist selected for this role will be determined based on various factors, including but not limited to scope of role, level of experience, education, accomplishments, internal equity, budget, and subject to Fair Market Value evaluation. The base salary range listed above is a good faith determination of potential base compensation at the time of this job advertisement and may be modified in the future.
