Job summary

MedStar Health is seeking a Community Health Program Coordinator to coordinate community health programming and health navigation efforts. Qualifications

• Bachelor's degree required
• 2 years related experience required
• Must obtain lay facilitator certification within 6 months of hire
• Certification as lay facilitator in health, wellness, fitness preferred
• Participation with a community health organization preferred
• Bilingual (Spanish speaking) preferred

Responsibilities

• Serve as a liaison between communities, individuals, and coordinated health care organizations.
• Communicate program purposes to clients and help identify socio-economic issues affecting health.
• Assist with health navigation and connections to primary care providers and specialists.
• Develop programming schedules, volunteers schedules, and room scheduling.
• Coordinate meetings, conferences, classes, seminars, and community activities for the department.
