Qualifications

• Bachelor's degree required.
• 2 years related experience required.
• Must obtain lay facilitator certification within 6 months of hire.
• Certification as lay facilitator in health, wellness, fitness preferred.
• Participation with a community health organization; experience as a lay facilitator in health/wellness programs preferred.
• Bilingual Spanish speaking preferred.

Responsibilities

• Serves as liaison between communities, individuals and health organizations; maintains relationships with community partners and manages program spaces.
• Informs clients about program purposes and health impacts; helps identify social determinants of health and connects to resources.
• Addresses barriers to care with culturally and linguistically appropriate health education; collects and reports program data.
• Assists with development of monthly programming and volunteers' schedules; supports curricula development and evaluation.
• Coordinates meetings, conferences and community activities; handles room scheduling, invites, minutes and communications; assists in volunteer selection and training.
