Job summary

MedStar Health is seeking a Community Health Program Coordinator to coordinate community health programming and health navigation efforts. Responsibilities

  • Serve as a liaison between communities, individuals, and coordinated health care organizations.
  • Communicate program purposes to clients and help identify socio-economic issues affecting health.
  • Assist with health navigation and connections to primary care providers and specialists.
  • Develop programming schedules, volunteers schedules, and room scheduling.
  • Coordinate meetings, conferences, classes, seminars, and community activities for the department.

Job Summary:

MedStar Health is looking for a Community Health Program Coordinator to join our team!The ideal candidate will bea lay facilitator withteaching skills to promote healthy behavior change.

TheCommunity Health Program Coordinatorwill coordinatecommunity health programming andassistwiththe directionof community-based health resource centers supported by MedStar Health. Incumbent will work alongsideregionalCare Transformation team to coordinate operations related to center programming and health navigation including planning, communication,logistics, and documentation.Will work closely with partners across the organization, system, and community including government agencies, faith-basedinstitutions,and non-profit organizations.

Primary Duties:
  • Serves as a liaison between communities,individualsand coordinated health care organizations. Builds andmaintainsrelationships with community partners.Maintains program equipment and wellness areas toprovidea safe, clean, and secure environment for members.
  • Communicates toclientsthe purposes of the program and the impact it may have on their wellbeing.Helps clientsidentifysocio-economic issues/SDOH that affect or contribute to their overall health and supports with connection to existing community resources to meet these needs as available.Assistsclients with healthcare navigation, includinglocatingprimary care providers, finding specialists, and attending outpatient appointments.
  • Identifiesand addresses issues that create barriers to care for specific individuals.Providesculturally and linguisticallyappropriate healtheducation.Collects, tracks andreportsprogram data to community site Director and to MedStar Health.
  • Assistswith the development of monthly programming schedules,volunteers’schedules, and room scheduling.Assistssite director and MedStar Care Transformation team with designing, implementing, and evaluating health-related curriculaappropriate torespective programs.
  • Coordinates and schedules meetings, conferences, classes, seminars, communityactivitiesand other related activities for the department. Ensure meeting readiness to include but limited to room availability, invitations, meeting requests and minutes, and communications.Assistsin preparation to include agendas and minutes.Assistsin theselection, training,orientingand assignment of volunteers in the health Center and its various programs. Mayassistwith development of standards of operations and performance management planning.
Qualifications:
  • Bachelor’s degree.
  • 2yearsrelated experience.
  • Must obtain lay facilitator certification within 6 months of hire.
  • Certification as lay facilitator in health, wellness, fitnesspreferred.
  • Participation with a community health organization. experience as a lay facilitator in health/wellness programspreferred. Bilingual (Spanish Speaking) preferred.
