Job summary

MedStar Health is seeking a Community Health Coordinator to join our team at Washington Hospital Center. The role supports planning, implementing, and coordinating community benefit initiatives under the Program Manager, serving as a bridge between community partners and hospital programs.

You will coordinate activities, document expenditures, support surveys, and work with physicians, program management, and workgroups to advance community programs and outreach.

Qualifications

• Bachelor's degree in Public Health, Social Work, Human Services, Psychology, Sociology required
• At least 3-4 years developing, implementing and tracking results of community outreach in a healthcare setting is required

Responsibilities

• Assists in coordinating planning and developing community benefits activities that support the hospital community health assessment implementation strategies
• Captures and documents all community benefit expenditures in the Community Benefit Inventory for Social Accountability (CBISA)
• Provides project planning support for implementation of the tri-annual Community Health Needs Assessment survey
• Support the planning and execution of programs tasks through coordination with physician liaisons, program management, and workgroups to support community programs and outreach opportunities
• Maintains comprehensive data tracking, including preparing and tracking correspondence, grant proposals, document and contracts

The Community Health Coordinator will work as a community liaison and serve as a bridge between community partners and hospital programs and associates.

• Assists in coordinating planning and developing community benefits activities that support the hospital community health assessment implementation strategies
• Captures and documents all community benefit expenditures in the Community Benefit Inventory for Social Accountability (CBISA)

This position has a hiring range of USD $59,820.00 - USD $101,836.00 /Yr.
