Job summary

Mass General Brigham Community Physicians in Massachusetts is seeking a community health education professional to work with individuals and communities to promote and maintain good health. The role focuses on outreach, education, and navigation of the healthcare system, under the supervision of a healthcare professional.

Hybrid work model with 3-4 days per week in the office or community in Hyde Park, Dorchester, West Roxbury, Wellesley, MA. 1-2 days per week remote work from home.

Qualifications

• High School Diploma or equivalent required.
• Associate's Degree or Bachelor's Degree in related field of study preferred.
• Experience in community outreach, health education, or related field preferred.

Responsibilities

• Conduct community outreach and engage community members to understand their health needs
• Provide individual and group health education and coaching to community members
• Assist community members in accessing and navigating the healthcare system, including providing referrals to appropriate health services
• Provide basic health screenings and assessments to identify health risks and provide appropriate interventions
• Assist in the development of health promotion programs and activities to address community health needs
• Collect data and maintain accurate records of community health outreach activities and interventions
• Attend meetings and trainings related to community health promotion and education
