Job summary

SGS Consulting seeks a community outreach specialist to engage with members on preventive health, care transitions, and chronic conditions. You will educate participants, assess needs, and coordinate referrals to services including behavioral health when appropriate.

You will build trusted relationships with community centers and providers, assist with Health Risk Assessments, and help members overcome barriers like transportation, knowledge gaps, and costs.

Qualifications

• Experience in health care related customer service or marketing environments.
• Bilingual in Spanish is preferred for outreach to the community.
• Proven ability to engage with diverse populations and coordinate access to services.

Responsibilities

• Initiate outreach to target preventive health and care transition initiatives in the community.
• Educate members on preventive health services and coordinate referrals as needed.
• Develop rapport with members and community centers to support health and wellness.
• Assist with Health Risk Assessments and navigate healthcare barriers.
