Community Health Centers of Benton and Linn Counties is seeking a Registered Nurse Care Coordinator to join a patient-centered team delivering preventive, team-based care across clinical, community, and home settings.

The role requires triage and nursing interventions to screen, assess, plan, deliver, and evaluate health services, with a focus on education, referrals, and applying standards of practice per Oregon rules.

Qualifications

• BSN and 1 year of nursing experience required.
• RN with 4 years of nursing experience may substitute.
• MSN and 8 years of nursing experience preferred.
• Current Oregon Registered Nurse license.

Responsibilities

• Triage to assess clients' health status for immediate care.
• Screen, assess, plan, deliver and evaluate nursing interventions.
• Provide education for care at home and coordinate referrals.
• Apply standards of practice per Oregon rules and county policies.
