Job summary

Community Health Centers of Benton and Linn Counties (CHC) seeks a Nurse Practitioner to provide high-quality primary care in a patient-centered, team-based model for a diverse patient population.

The role supports 700–1200 patient panels, sees about 14 patients daily, and offers opportunities to teach medical residents and students. This intermittent position emphasizes prevention, chronic disease management, and coordinated care in Benton County, Oregon.

Qualifications

• Master's of Science in Nursing from an accredited college or university.
• Currently licensed as a Nurse Practitioner in the State of Oregon, or in a different state with the ability to obtain an Oregon license.

Responsibilities

• Provide high quality primary care in a patient-centered, team-based model.
• Manage panels between 700 and 1,200 patients and see about 14 patients per day.
• Participate in after-hours calls with nurse triage support.
• Opportunity to teach medical residents and students from local institutions.
