Job summary

The Primary Health Network is seeking a Community Health Nurse (RN) for hands-on home visits across Mercer and Lawrence Counties, PA, with occasional travel to nearby areas. You’ll educate patients, coordinate care, and work with care teams to improve outcomes.

Strong nursing assessment, documentation, and travel readiness are essential, along with CPR certification and a valid driver’s license. This role emphasizes patient education and community resource navigation.

Qualifications

• Graduate of an approved school of nursing with current, unrestricted Pennsylvania and Ohio RN licenses.
• Current CPR Healthcare Certification and strong nursing assessment and clinical decision-making skills.
• Ability to work independently, travel to patient homes, and adapt to varied community settings.
• Strong communication, patient education, organization, and culturally responsive care skills.
• Valid driver's license and acceptable driving record; knowledge of immunization and vaccine-handling practices.

Responsibilities

• Conduct home nursing visits, assess patient needs, and provide direct nursing care and health screenings.
• Educate patients and families on chronic disease management, medications, prevention, and self-care.
• Coordinate care, assist with patient navigation, and connect patients to community resources.
• Identify barriers to care, including transportation and health literacy, and develop individualized support plans.
• Collaborate with providers, Community Health Workers, and care teams; document services accurately in the EHR.
• Support outreach, wellness programs, immunizations, and testing in accordance with PHN policies.
