The Primary Health Network is seeking a Community Health Nurse to promote wellness through education, screenings and direct care. This role focuses on reducing health disparities by addressing environmental, physical, and social determinants of health in the community.
You will collaborate with diverse populations, provide immunizations per CDC/CDPH guidelines, and contribute to outreach and advocacy initiatives while maintaining comprehensive patient-centered care.
Community Health Nurse (Full Time )
Location: Clarion County
A Community Health Nurse works to promote the overall wellness of individuals and communities through education, advocacy and delivery of care.The CHN promotes healthy lifestyle choices by instituting health screenings, wellness projects and provides education on disease management and prevention.
Developing relationships helps the CHN address health disparities within marginalized communities by focusing on the environmental, physical, and social determinants of health. The CHN also builds individual and community capacity through cultural mediation and by increasing the knowledge and self-sufficiency through a range of activities such as outreach, community education, and advocacy.
The general office hours for this position are:
Monday 8:30am-5:00pm
Tuesday 8:30am – 5:00pm
Thursday 8:30am – 5:00pm
Friday 8:30am 5:00pm
Graduate of an approved school of nursing
Current unrestricted PA and Ohio RN license
Knowledge, skill and ability to provide routine immunizations based on CDC and CDPH recommendations
