Job summary

Renown Health seeks a Community Health Worker to support patient and member populations with social determinants. You will assist the care team in achieving health outcomes, advocate for patients, and help navigate healthcare systems and community resources.

This role documents activities in the EMR, communicates HIPAA-compliantly, and coordinates non-clinical case management, including referrals, transportation, and follow-up.

Qualifications

• Must be able to read, write and speak English.
• AA or AS degree preferred; health-related degree preferred.

Responsibilities

• Support patients with social determinants to improve health outcomes.
• Navigate healthcare and connect patients to community resources.
• Document activities in the EMR and ensure HIPAA compliance.
• Coordinate non-clinical case management, referrals, and transportation.
• Engage with patients in-person to build trust and promote engagement with care.
