Job summary

Memorial Hermann is seeking an entry-level Community Health Worker to conduct NMDOH screenings and guide patients to community resources and medical homes. The role is on-site at a Memorial Hermann campus and supports remote engagement via digital tools, with occasional travel to clinical or community settings.

The CHW will document social needs, build trust with diverse populations, and coordinate with care teams to ensure comprehensive support while reporting to a manager within the Community

Qualifications

• High school diploma or GED required.
• CHW certification preferred (TDSHS).
• Bilingual Spanish preferred.
• Experience in healthcare, social services, customer service, or community outreach is a plus.
• Strong oral and written communication skills; ability to build trust across diverse populations.
• Ability to use digital platforms and technology for remote engagement and documentation.

Responsibilities

• Conduct screenings to identify NMDOH and document risk levels in the EMR.
• Use motivational interviewing to explore needs and connect to resources.
• Verify referrals with partner agencies to ensure alignment with needs and eligibility.
• Manage scheduling and follow-ups; arrange appointments and verify completion.
• Conduct follow-up outreach on referrals or appointments to ensure unresolved needs are addressed.
• Participate in community activities to identify new resources for navigation efforts.
• Provide support at other Memorial Hermann locations as needed.
• Maintain safety, adhere to policies, and support budget/time management goals.
• Promote professional growth and serve as a mentor or resource to others.
• Model Memorial Hermann’s standards of safe, compassionate, and efficient care.
