Memorial Hermann Community Resource Centers across the Greater Houston area are seeking a Community Eligibility Specialist to screen NMDOH and navigate clients to community resources and medical homes, ensuring eligibility and enrollment for SNAP, Medicaid, and utility assistance. The role emphasizes accurate documentation and follow-up to verify service access.

Strong communication, organizational skills, and comfort with digital tools are essential; bilingual Spanish is preferred.

Qualifications

• Education: High school diploma or GED.
• Licenses/ Certifications: CHW/TDSHS options or CHW-I/CHW-E/CHW-T.
• Preferred: CAC, BEC, Texas Eligibility Worker Certification.
• Two years in healthcare, social services, or community outreach preferred.
• Strong communication, bilingual Spanish preferred.

Responsibilities

• Conduct NMDOH screenings and document risk levels in the EMR.
• Guide clients through SNAP, Medicaid, utility and other program applications with accurate documentation.
• Follow up to confirm enrollment and service access, documenting activities.
• Collaborate with Memorial Hermann care teams to provide medical and non-medical support.
• Participate in community activities to identify new resources and represent Memorial Hermann at events.
• Maintain adherence to policies, procedures, and quality service standards.
