Job summary

Memorial Hermann Health System seeks a Patient Care Navigator to provide in-person support in emergency, inpatient, and outpatient settings. You will identify NMDOH drivers, apply motivational interviewing, and help patients transition to stable community care, coordinating with social workers and case managers to arrange resources at discharge.

You will document interactions in EPIC, monitor NMDOH indicators, and support reductions in ER visits and readmissions for vulnerable patients.

Qualifications

• High School or GED required; Associate degree preferred.
• Minimum 3 years in healthcare/hospital settings.
• Bilingual Spanish strongly preferred; EPIC experience preferred.

Responsibilities

• Screen for NMDOH and document risk levels in EMR.
• Use motivational interviewing to engage patients in behavior change.
• Collaborate with care teams to develop discharge plans.
• Refer to internal/external resources and track outcomes.
