Sanford Health in Marshfield, WI is hiring a Community Health Worker (CHW) to bridge underserved communities with health and social services within a team-based care model. The CHW will provide information, advocacy, and navigation support to help patients access needed resources.

Qualifications

• High school diploma or equivalent required; college courses strongly preferred but not required.
• Minimum of three years ambulatory care experience and/or community outreach experience preferred.
• Motivational Interviewing skills required with training to be completed within six months of hire.
• Certificate from a Community Health Worker program required upon hire or within six months.
• Must have a valid driver’s license.

Responsibilities

• Serve as a bridge between under-served communities and health and social service systems.
• Assist patients in navigating a complex health and human services system.
• Provide information, advocacy, and support to help patients access services.
• Present to community groups and engage in public speaking.
• Collaborate with an inter-professional team to set goals and plans of care.
